Ebenconcepts has negotiated exclusive contracts with several national carriers. Our customers can take advantage of our exceptional relationships with these carriers through increased benefit limits and reduced plan minimums.
The products available include:
- Voluntary life
- Guarantee issue coverage of $150,000 for eligible employees (maximum of five times salary)
- Guarantee issue coverage of $50,000 for eligible spouses
- Guarantee issue coverage of $10,000 for eligible dependent child(ren)
- Voluntary AD&D
- Guarantee issue coverage of $150,000 for eligible employees (maximum of five times salary)
- Guarantee issue coverage of $50,000 for eligible spouses
- Guarantee issue coverage of $10,000 for eligible dependent child(ren)
- Voluntary dental
- Participation minimum reduced to only five employees for a PPO plan
- Participation minimum reduced to only three employees for a DHMO plan
- Supplemental coverage (including long-term care and catastrophic illness plans)
- No minimum participation requirements
